This normally happens, if you forget to give the right size with the banner. OpenX not always recognizes the right size of a banner. Simple edit the banner and add the width and height.

Thanks for taking the time to note this - surely it will help someone out there.
We have tried to cover this with:

Local banners should detect the size fine however when creating an external or HTML/text banner you need to define the size yourself.
